If you have everything you need to participate in the Perkins Experience survey, you can now proceed with the questionnaire itself.
- To start, go to the official Perkins survey web page, www.perkinsexperiencesurvey.com
- Check your Perkins receipt. You should have a 15-digit survey code written on it. Write that code in the blank field on the main page of www.perkinsexperience.com
- If you do not have a survey code on your receipt, you can press the second click here link on the Perkins survey page. In this case, you will need to enter the Perkins restaurant number, check number and the date printed on your receipt from the three drop down menus. All of these details should appear at the top of your receipt.
- Press the green Start to continue.
- Go through all the questions in the Perkins Experience Survey. You must answer all the questions to complete the survey unless the question is marked “Optional”
- At the end of the survey, you will receive your Perkins survey validation code. Write this code on your Perkins receipt, in the field underneath “Validation Code”. Bring this receipt with you the next time you visit a Perkins restaurant to benefit from the discount.
