- Be at least 18 years old.

- Type the restaurant number from the top of your receipt into field number one.
- Select the date and time of your visit from the menus.
- Write your receipt number and the amount you spent during your visit into the correct areas.
- Click the green “Enter” button.
- Choose whether you agree or disagree with various statements about how your visit to Triple O’s went.
- Click the green “Next” button to go to each new page after you finish all questions on the previous page.
- Enter your name, email address, and phone number, so you can be contacted if you win the sweepstakes.
- Submit the survey by clicking the next button.
